ADMINISTRATIVE DESCRIPTION: Substance abuse treatment center offers outpatient and residential programs. Residential program is long term, three to six months, and includes a special program for women and children. Also provides a therapeutic shelter for substance abusers ages 18+ who are not in need of immediate medical or psychiatric care. Maximum length of stay is ten days.
HIV Prevention Case Management (PCM)
AIDS/HIV PREVENTION COUNSELING/CASE/CARE MANAGEMENT
HIV prevention case managers work with people who are at high risk of HIV infection by providing individualized counseling on risk prevention and coordination of services needed to meet a client's basic needs in order that the client can better focus on developing behaviors to protect his/her health. PCM is intended for individuals with multiple complex problems and risk reduction needs who are having or are likely to have difficulty initiating or sustaining practices that reduce or prevent HIV acquisition, transmission, or re-infection.
ELIGIBILITY: Partners of persons who have HIV or AIDS; Injection drug users; Men who have sex with men; Persons repeatedly infected with sexually transmitted diseases (STDs); Commercial sex workers or those who exchange sex for drugs or money; Persons recently released from prison with a history of substance abuse; HIV discordant couples with inconsistent condom use; HIV positive persons who need assistance in preventing transmission to others
