ADMINISTRATIVE DESCRIPTION: Community action agency provides community based services for economically disadvantaged people to promote self sufficiency and economic independence. New Opportunities programs serve towns in the Central Naugatuck Valley, Litchfield Hills, and greater Meriden.

Ryan White Title I: Case Management, Transportation, EFA
CASE/CARE MGMT/TRANSPORTATION/TEMP. FINANCIAL AID
Ryan White eligible clients receive case management services, transportation service for medical appointments and appointments with social service agencies, and limited financial assistance for emergency needs.
FEE: No fee
APPLICATION: By appointment only; 48 hours advance notice for transportation service
ELIGIBILITY: Persons with HIV/AIDS; Age 18+; Below 300% of FPL
