Working for a healthier future for Waterbury”

Here at the Waterbury Health Access Program, we are dedicated to providing an innovative, integrated system to serve the medical needs of the uninsured and underinsured patients in the greater Waterbury area. Founded in 2003, by a consortium of health care providers, our multifaceted program provides eligible patients with access to affordable health care coverage, prescription medications, disease management, and social services. As part of this initiative, we are also implementing a community-wide paperless electronic medical record (EMR) system with a powerful patient data sharing tool to assist providers in getting access to clinical information.

Case Management:Over 4200 patients are receiving assistance with obtaining health care coverage, prescription medications, and health care navigational services. An internet-based case management system is under development.

Disease Management:Over 500 patients are receiving HIV and Diabetes disease management services.

Waterbury Project Access:Over 330 community providers have agreed to participate in a coordinated charity care program where eligible patients receive care at no cost.

Waterbury Health Information Network:Two of the consortium members have implemented the EMR, and the data sharing tool is operational in a testing database. This provides the basis for forming a nationally-recognized Regional Health Information Organization (RHIO).
